Recommendations for soil test results?

I have Bermuda Tiff 419 near Dallas, TX. What products should I be looking for? Granular or liquid spray? Also, am I correct that I should be putting these rates down all at once for the first application?

The email states:

The way our reports are set up is that the fertilizer recommendation to the right of the results is the current need. The current need will include the first nitrogen application and all the phosphorus, potassium and other nutrients you may need.
Then, if needed, addition nitrogen recommendation at the bottom is for future applications of nitrogen. This is to be applied in four to six weeks and repeated as long as you need continued growth.

You are looking for granular fertilizer. Here are the soil remediation guidelines:

The general rule is to put down no more than 1 lb of a nutrient per 1000 sq in an application. You need three applications of phosphorus. See if you can find a fertilizer listed under the phosphorus heading in the guidelines. You will also need probably four applications of nitrogen.
